Special Requirement 1: You need to attach a copy of the decision that you are requesting to be reviewed.

Special Requirement 2: the applicant must serve a copy of the application on each other party to the proceedings and any other person who is entitled to a notice of the application under the Act.

An application for review must be made within 28 days after the decision is made or the reasons for the decision are given to a person.

Special Requirements:

An appeal under Section 116 must be filed within 14 days after notice of the Board’s decision is given to the person.
